If you are unable to submit electronically please print for completion and bring with you on the day of your office appointment. Completing patient forms prior to your visit will enhance the registration process and assist in an efficient check-in the day of your appointment.

Please note that you may be required to complete additional forms upon arrival.

Preparation Items

On the day of your appointment there are several items needed to successfully complete the check-in process:

  • Insurance card (cards)
  • Photo identification
  • List of all current medications, including dosage and frequency
  • Pharmacy name, address and telephone number
  • Valid Referral (If your insurance requires a referral, please know it is patient responsibility)
  • Co-payments/co-insurance to be paid at the time of service

We kindly ask that you arrive 15-20 minutes before your scheduled appointment time to allow us to properly register you for your appointment. All necessary forms and check-in procedures must be completed before you can be seen by a provider. For this reason, completing the available paperwork prior to your visit is advantageous.

Patients arriving late for scheduled appointments may be required to reschedule.
